For Primary Healthcare Providers
Download the referral form, complete the provider section on page 1, and return it by email to your patient’s nearest East Coast Sleep Clinic location. We’ll take care of arranging the home sleep test from there.
For Patients
Download the form and complete page 2, then bring it to your doctor or nurse practitioner to fill in the referral on page 1. Alternatively, you’re welcome to complete page 2 and email it directly to your nearest clinic — we’ll follow up with your healthcare provider on your behalf.
